We welcome professional referrals by telephone or in writing. For all our services, we have standard referral forms that enable us to get the information we need to assess the needs of the individual and whether we're the most appropriate agency to support them.
Our referrals process 
Our referral forms are available from our Lowestoft office. We can accept telephone referrals if you can give us all the relevant information to complete the referral form. After a referral, a Care Coordinator will assess the client's needs and allocate them to a worker in our team. This worker will then make contact with the client and the referrer and arrange an initial appointment.
